where educators come to learn from one another

I have purchased 2 NOOKS for student use in a high school and would like to have the students sign them out. I am looking for samples of liabilty forms, so if you have one and wouldn't mind sharing, I will appreciate it. I am worried that the student will not return it and I will have no recourse. (grades are online for parents, can't hold school records, can charge them, but few pay...) The students are notorious for not returning books or moving in the dark of night. The money for the NOOKS was raised through a Barnes and Noble Book Fair, so I am excited to offer this ereader to the studnets, but want the process to be smooth and fair.
